The two-year pilot uses federal ARPA funds for primary care services from University of Tennessee Health Science Center (UTHSC) coaches. Complimentary screenings are followed by personalized treatment plans and referrals to community providers. Representatives from the county's health and community service teams connect patients with relevant services. ShelbyCares on 3rd also hosts meal planning and fitness courses to support improved health outcomes.
